Integrating CRM with Forms Created Using Elementor

> **Before you begin:** This module must be enabled first. Go to **Settings → System Upgrade** > and activate **Integration with Elementor Pro Forms**, then return to this guide. The **Elementor Pro** > plugin must also be installed and active on your site — this module adds a new action to Elementor Pro's > own Forms widget.

Module Overview

The **Elementor Pro Forms Integration** module connects any form you build with the **Elementor Pro** Form widget directly to your CRM. Every time a visitor submits a connected form, the CRM automatically:
  • Saves the full submission (every field the visitor filled in).
  • Tries to match the visitor to an existing CRM **user** or **lead** using their email or phone number.
  • Optionally creates a new **lead** automatically when no match is found.
  • Makes the submission visible in a dedicated list inside the CRM, and on the matched person's profile.

What you can do with this module

  • Turn any Elementor Pro form into a CRM data source with a single action added inside the Elementor editor — no code required.
  • Automatically link form submissions to the correct existing user or lead.
  • Automatically create a new lead from a submission when the visitor is not yet in your CRM.
  • Attach up to three custom pieces of information (meta key/value pairs) to auto-created leads.
  • Browse, search, and filter every submission per form in its own CRM page.
  • Mark submissions as read, star important ones, and remove submissions you no longer need.
  • See a visitor's full form-submission history directly on their user or lead profile page.

Typical use cases

  • A "Contact Us" form on your website automatically creates a lead for every new visitor who writes in.
  • A "Request a Callback" form matches the phone number to an existing customer and adds the submission to their profile history, so the sales team sees it without asking again.
  • A landing-page signup form is checked for spam and duplicates — the team stars important submissions and marks the rest as read.

Connecting the CRM to an Elementor Form

https://VIDEO_PLACEHOLDER_URL
Once the module is active, every **Form** widget you edit in Elementor Pro gains a new action you can attach to it.
  1. Open the page containing your form in the **Elementor editor**.
  2. Select the **Form** widget and go to the **Actions After Submit** setting.
  3. Check the **LoyalAxis CRM** option.
  4. A new settings section named **LoyalAxis CRM** appears in the panel. Configure it as described below.
  5. Click **Update** (or **Publish**) to save the page.

LoyalAxis CRM settings

SettingDescription
**Enable CRM Integration**Turns the whole integration on or off for this form. Default: **On**. If turned off, submissions are not sent to the CRM at all.
**Auto-create Lead**When **Yes**, the CRM automatically creates a new lead if the submission does not match any existing user or lead. Default: **No**.
**Phone Number Field**Choose which field of your form holds the visitor's phone number. The dropdown lists every field currently in the form and updates automatically as you add, rename, or remove fields.
**Email Field**Choose which field of your form holds the visitor's email address. Same dynamic dropdown behavior as the phone field.
**Name Field**Choose which field of your form holds the visitor's full name. Same dynamic dropdown behavior as the phone and email fields. Used to name an auto-created lead — see [How Submissions Are Matched to Users and Leads](#how-submissions-are-matched-to-users-and-leads).
**Meta Key 1 / Meta Value 1**An optional custom label and value saved on the lead when it is auto-created (for example, Key: `Source`, Value: `Landing Page A`).
**Meta Key 2 / Meta Value 2**A second optional custom label/value pair.
**Meta Key 3 / Meta Value 3**A third optional custom label/value pair.
The **Phone Number Field**, **Email Field**, and **Name Field** dropdowns all show **"— Select Field —"** until you pick a field. Fields such as reCAPTCHA, file upload, hidden fields, dividers, or step breaks are not shown in these dropdowns, since they can never hold contact information.
Leave the meta key/value fields empty if you do not need to store extra information on auto-created leads.
> **Note:** Mapping the phone or email field is optional, but without at least one of them the CRM cannot match or auto-create anything — the submission is still saved, but with no linked user or lead. The **Name Field** is optional too; without it, an auto-created lead is simply named after the submitted email or phone number.

How Submissions Are Matched to Users and Leads

https://VIDEO_PLACEHOLDER_URL
Every time a connected form is submitted, the CRM follows these steps automatically:
  1. **Look for an existing user.** The CRM checks if the submitted email or phone number belongs to an existing WordPress user account. If found, the submission is linked to that user.
  2. **Look for an existing lead.** If no user is found, the CRM checks if the email or phone number belongs to an existing lead. If found, the submission is linked to that lead.
  3. **Create a new lead (optional).** If nothing matches and **Auto-create Lead** is turned **On** for the form, a new lead is created automatically. Its name comes from the submitted **Name Field**, if one is mapped — otherwise from the submitted email (or phone number, if no email was submitted). Any configured Meta Key/Value pairs are saved on the new lead as well.
  4. **Save the submission.** Whether or not a match was found, the full submission — every field the visitor filled in, plus their IP address and submission date — is always saved and appears in the form's submission list.
If a submission cannot be matched and **Auto-create Lead** is off, it still appears in the submission list, simply without a linked **User** or **Lead**.

Splitting the name into first and last name

If the value submitted in the **Name Field** contains two or more words (for example, `Jane Doe`), the CRM splits it automatically: the first word becomes the lead's **First Name** and the rest becomes the **Last Name**. Both are saved on the lead in addition to its full name.
If the submitted value is a single word (for example, just `Jane`), it is used only as the lead's name — no separate first/last name values are saved.

The Elementor Forms Menu

https://VIDEO_PLACEHOLDER_URL
Once a connected form receives its **first submission**, an **Elementor Forms** section automatically appears in the CRM sidebar menu, with one menu item per form, named after the form itself.
> **Note:** A form only appears in this menu after it has received at least one submission. Connecting a form in Elementor is not enough by itself — submit the form once (or wait for a real visitor to do so) to see it listed here.
Click any form's name in the menu to open its submission list.

Submission List Page

https://VIDEO_PLACEHOLDER_URL
Each form has its own list page showing every submission received for that form.

Search and filters

FieldDescription
**Quick Search**Searches across the email, phone number, IP address, and every field value submitted in the form.
**Status**Filter by **Unread**, **Read**, or **Starred**.
**From Date** / **To Date**Filter submissions by the date they were received.
Above the search bar, filter tabs let you jump straight to **All**, **Unread**, **Read**, or **Starred** submissions, each showing a live count.

Table columns

ColumnDescription
**User**Link to the matched WordPress user's CRM profile (shown only if the submission is linked to a user).
**Lead**Link to the matched lead's CRM profile (shown only if the submission is linked to a lead).
**Date**The date and time the form was submitted.
*(one column per form field)*Every field from the actual Elementor form appears as its own column, named after the field's label. Any web links inside a field's value are shown as clickable links.
**IP**The visitor's IP address at the time of submission.
**Status**A **Read** or **Unread** badge.
**Actions**See below.

Row actions

  • **View** (eye icon) — opens the full submission in a detail window and marks it as read.
  • **Open Page** (external-link icon) — only shown when the submission's page is known; opens the page the form was submitted from in a new tab.
  • **Star** (star icon) — click to mark a submission as important. Click again to remove the star. The tooltip reads **"Click to Star"** or **"Click to Unstar"** depending on its current state.
  • **Remove** (trash icon) — permanently deletes the submission after a confirmation prompt. Only visible to users with permission to delete submissions.

Viewing a Submission

https://VIDEO_PLACEHOLDER_URL
Clicking the **View** action on any submission opens the **Message** window, showing:
  • The linked **User** or **Lead** (as a clickable link to their profile), if any.
  • The submission **Date**.
  • The visitor's **IP** address.
  • Every field the visitor filled in, with its label and the value they entered.
Opening this window automatically marks the submission as **Read**.

Bulk Actions

https://VIDEO_PLACEHOLDER_URL
Select the checkboxes next to one or more submissions to reveal the bulk action bar above the table.
ActionDescription
**Remove**Deletes all selected submissions at once. Requires delete permission.
**Add to Target Group**Adds the people linked to the selected submissions to a target group for marketing or follow-up campaigns. Only available if the **Target Group** module is active. If no target group exists yet, you will be prompted to create one first.
  1. Check the boxes next to the submissions you want to act on (or use **Select All** in the header).
  2. Choose an action from the **Bulk Action** dropdown.
  3. Click **Run**.

Submission History on Profiles

https://VIDEO_PLACEHOLDER_URL
Open any **user** or **lead** profile in the CRM and look for the **Elementor Forms** tab. This tab shows every form submission linked to that person — matched either by their email, phone number, or their profile ID — no matter which form it came from.
The tab shows a count badge with the number of submissions, and a table with:
ColumnDescription
**Form**The name of the form that was submitted.
**Date**When it was submitted.
**IP**The visitor's IP address at submission time.
**Status****Read** or **Unread** badge.
**Actions****View** to open the submission detail, and the **Star** toggle.
> **Note:** Only submissions for forms you have permission to view appear on the profile, even if the person submitted more forms than are shown to you.

Permissions and Roles

Access to this module is controlled by capabilities assigned to user groups, listed under the **Elementor Forms** permission group.
CapabilityWhat it allows
**Permission to view messages of *(form name)***One capability per connected form. Grants access to that specific form's menu item and submission list.
**Permission to delete Elementor form submissions**Allows removing submissions individually or in bulk, and shows the **Remove** action.
**Permission to view the history of Elementor form submissions in the user profile**Allows the **Elementor Forms** tab to appear on user and lead profiles.

Setting permissions

  1. Go to **Settings → Groups** (or your user permissions panel).
  2. Select the group you want to configure.
  3. Find the **Elementor Forms** section in the permissions list.
  4. Check or uncheck the capabilities as needed — including the specific forms each group should be able to see.
  5. Save changes.
Users without a form's specific view permission never see that form's menu item, submission list, or its entries on a profile.

Practical Examples

Example 1 — Website contact form

**Scenario:** You have a "Contact Us" form on your homepage and want every message to create a CRM lead automatically.
**Setup:**
  • **Enable CRM Integration:** On
  • **Auto-create Lead:** Yes
  • **Email Field:** mapped to the form's email field
  • **Phone Number Field:** mapped to the form's phone field (if present)
  • **Name Field:** mapped to the form's name field (if present)
**Result:** Every new visitor who fills in the form and is not already a user or lead gets added as a lead, named after what they typed in the name field (split into first/last name when it has two words), with the message saved and visible on their profile.

Example 2 — Existing-customer callback request

**Scenario:** A "Request a Callback" form is used mostly by existing customers, and you do not want new leads created for every random submission.
**Setup:**
  • **Enable CRM Integration:** On
  • **Auto-create Lead:** No
  • **Phone Number Field:** mapped to the phone field
**Result:** If the phone number matches an existing user or lead, the request appears on their profile automatically. If it doesn't match anyone, it still appears in the form's submission list for manual review, but no new lead is created.

Example 3 — Tagging the lead source

**Scenario:** You run several landing pages and want to know which page each auto-created lead came from.
**Setup:**
  • **Auto-create Lead:** Yes
  • **Meta Key 1:** `Source`
  • **Meta Value 1:** `Landing Page — Summer Promotion`
**Result:** Every lead created from that specific form carries the `Source` information, making it easy to identify later.

Example 4 — Naming leads properly from a single "Full Name" field

**Scenario:** Your form has one text field asking for the visitor's full name, and you want auto-created leads to have a proper first and last name instead of just an email address.
**Setup:**
  • **Auto-create Lead:** Yes
  • **Name Field:** mapped to the "Full Name" field
**Result:** A submission of `Jane Doe` creates a lead named "Jane Doe" with **First Name** `Jane` and **Last Name** `Doe` saved on it. A submission with just one word, like `Jane`, creates a lead named "Jane" with no separate first/last name saved.

Best Practices

  • **Always map at least the email or phone field.** Without one of them, submissions cannot be matched or turned into leads.
  • **Map the Name Field whenever your form asks for the visitor's name.** It gives auto-created leads a proper name (and first/last name) instead of just their email address.
  • **Turn on Auto-create Lead only for forms meant to bring in new contacts.** For forms mostly used by existing customers, leave it off and review unmatched submissions manually.
  • **Use the Status filter regularly.** Check the **Unread** tab to make sure no new submission goes unnoticed.
  • **Star anything that needs follow-up.** It keeps important submissions easy to find later, separate from routine ones.
  • **Assign form-specific permissions carefully.** Grant each team only the forms relevant to their work, especially when several departments manage different forms.
  • **Remember new forms need a first submission to appear in the menu.** Test a connected form once after setting it up so it shows up in the CRM.
